The adverb “always” should be used before the verb.

The word modifying the verb “use” should also be an adverb. So “cleanly”, not “clean”.

What is being used? “Use” is a transitive verb (他動詞), so there must be an object. For example, “this restroom.”

“Thank you for always using this restroom cleanly” is a correct sentence grammatically, but I don’t think it’s very natural. In America, this sign would say:

O Thank you for keeping this restroom clean.
